Do I qualify For a Home Visit?

If any of these apply to you, you may qualify:

1. Your homebound

2. have COPD or CHF with SOB and difficulty with stamina

3. At risk for falls and have an orthopedic or neurological condition

4. Use an assistive device such as walker, cane or wheelchair.

5.Require help of another person to leave the house.

6. Have confusion and poor memory to drive.
